Once you’ve submitted the form below:

• An Admissions Coordinator will reach out to the client to walk them through the admission process. If you would like us to reach out to you first, please note it in the Reason for Referral field.
• If you have clinical records related to the referral, those can be faxed to 424.416.7238 or emailed to OBH-OP-Admissions@odysseybh.com. Clinical records should include the client face sheet, insurance information, current clinical evaluation, three most recent progress notes, medication list, and discharge plan.
